Dr Francis Palmer has over 25 five star reviews on Yelp but for some unknown reason, Yelp chooses to arbitrarily hide all but two of these from the public.  We believe that you have the right to read real reviews from actual patients instead of just the ones that Yelp decides you have the right to see.  Perhaps it’s time for you to stop trusting Yelp and the reviews placed there and find an “honest” review site.

Dr. Palmer and his wife Robyn are just simply amazing; there’s no other word for it.
I did a ton of shopping around and information gathering when I began exploring anti aging options in my 40’s. I have never wanted that “altered” look; I just wanted to look great for my age and continue to do so. I was terrified to end up with cheeks that look like I’m storing nuts for winter…I see so much of that and it’s hard not to stare at what was probably a really nice natural looking person at one point-

Dr. Palmer was the only plastic surgeon/or assist who did not try to talk me into procedures I honestly didn’t need. I had suggestions from other surgeons about cheek bone implants and lip implants and whatnot and Dr. Palmer was the first person who explored the existing beauty and symmetry that I was born with and just helped me to capitalize on this. He is truly an artist at balancing your look and features without making you look “not like you” lol

His bedside manner and wife Robyn; co pilot/head nurse are amazingly kind, gracious people and make me feel like they are welcoming me into their home every time I see them.

Dr Francis Palmer’s latest Yelp Review.  Take a look at why Dr Francis Palmer was voted One of the “World’s Best” Plastic and Cosmetic Surgeons.  This latest yelp review is one of over 25 5 star Yelp reviews that Yelp for some confusing reason chooses not to show you but attempts to hide.  Makes you wonder what is Yelp’s reason for hiding an obvious happy patients 5 star review.  This seems to show a lack of honesty and trust and calls into question why you would trust what you read on Yelp.  Yelp says it wants you to trust it but why are they hiding this review and 24 other 5 star reviews just like it from you?  Perhaps it’s time to choose another source for reliable reviews.

Dr. Palmer is simply amazing. He literally took 15 years off my face WITHOUT anybody knowing I had plastic surgery. I’m a 50 year old man….and I am really enjoying meeting beautiful woman who would not pay me attention but for his “magic”…I’m telling you the guy is an artist. A true artist. Would you trust somebody to work on your face who couldn’t do a sculpture? Look at his artwork…look at his credentials…and trust me here. I never thought I’d be one to go to a “plastic surgeon”…Best thing I’ve ever done for my looks, confidence and self esteem. I get compliments every day now – literally every day. That didn’t happen before. If you can afford him…he is the only choice. Best $ I ever spent on myself. I suspect those that complained about him had unrealistic expectations …he’s a doctor ….ya gotta have something to work with! I have never been so grateful to somebody I had to pay so much money to! But ya know…he is worth whatever he asks… That’s a fact.

Facelifts versus Non Surgical Facelift by Francis R Palmer MD, world renowned beverly Hills Plastic and Cosmetic Surgeon voted One of the “World Best” plastic and Cosmetic Surgeons discusses the various options and compares how they stack up to one another.  With all the non surgical skin tightening options and some “hyped” and frankly downright misleading procedures out there today, how is one supposed to know how these compare to a Facelift.  Good question and to answer it, let’s first take a look at each procedure and examine what it really is and does in the way of rejuvenating the face.

Whether it’s a non surgical or a surgical facelift the procedure should impact the following areas of facial tissues to affect real, meaningful change.  The more skin and SMAS laxity, the more need for removal of the excess skin and tightening of the underlying SMAS muscle layer.  Remember that skin removal can only occur when an incision is placed around the front of the ear, preferably contoured to the ear, as this is the direction of skin elevation and pull.  Any procedure lacking this incision is incapable of removing excess skin, which means that whatever method of action the procedure uses must create adequate skin tightening and shrinkage.  Placed in the proper perspective, a facelift removes 2 inches of skin or more while most non surgical procedures tighten and shrink the skin several mm’s.  There are 25 mm’s in 1 inch and 50 mm’s in 2 inches.  So that means over 50 mm’s of skin can be removed with a facelift vs a few mm’s with the non surgical options.  Keep that in mind when comparing the various claims being made by the non surgical procedures.

The common forms of Face Lifts include:

  • Most Popular are short incision Face Lift variants with a limited incision, limited tissue dissection yet the ability to tighten the underlying SMAS muscle, remove excess fatty tissue and skin all in a 90 minute procedure with a recovery time measured in days.  Other traditional facelifts do something similar but require longer incisions, surgery and recovery times measured in weeks or months.

The common forms of Non Surgical Skin Tightening Procedures:  ***Some claim to be non surgical facelifts but do not mer the minimum requirements of all facelifts which are SMAS layer tightening, excess fat and skin removal.

  • Protege RF skin tightening system.  This is the non surgical procedure that we offer.  It uses radio frequency waves (RF) that penetrate into the layers of the skin stimulating collagen production leading to smoother, tighter and youthful looking skin.  There are other RF systems but they hurt (my opinion) and some have been associated with dissolving facial fat which may not be desired.
  • Ulthera uses ultrasound to build collagen within the skin for similar results as the Protege but has more discomfort associated with it vs the Protege.
  • The “Vampire” lift is a hyped, marketing ploy in my opinion.  It uses platelet rich plasma (PRP) which is a blood component derived from spinning down blood in a centrifuge.  The PRP is the injected into the face under the skin.  PRP does not have the ability to tighten the SMAS, shape or remove excess fat, remove excess skin or tighten the skin through collage formation.  In other words, none of the basic requirements of a proper, effective facelift.  There might be some temporary facial filling associated with this procedure but this wouldn’t be expected to last.
  • The “Stem Cell” Facelift is another hyped, marketing procedure in my opinion that is a fancy new name for fat transfer, which has been around for over 25 years with very mixed reviews despite some zealot advocate claims to the contrary.  The procedure is basically fat removal with processing before being re-injected into the face = fat transfer.  While it is true that fat does contain minuscule numbers of adult stem cells, these are nowhere near the concentration needed to do something remarkable like replace old sagged muscle with new muscle cells or to rejuvenate the collage layers within the skin.  This would require a 4-6 week incubation of the stem cells in a nutrient rich environment where the cells could multiple into huge numbers before being re-injected into the face.  This procedure would be regulated and would have to be approved by the FDA that oversees all stem cell treatments within the US.  The fact that this procedure does not carry FDA approval says it all.  It is not what it claims to be and is merely fat transfer that like PRP does not have the ability to achieve any of the basic requirements of a facelift.

So the take away from all this is to be aware of what the various procedures can truly accomplish and choose accordingly.  If you have loose skin and prominent jowls, a facelift would be far more effective.  If you do not want surgery and are willing to have your “next best” option with the understanding that the results are not equal or equivalent then I would think a reliable skin tightening technology using RF or ultrasound might make sense.  If you don’t like any pain, I’d suggest the Protege which is typically performed as a series of five, twenty minute treatments spaced 1 -2 weeks apart for maximum collagen building.

A Gummy Smile is over-exposure of the upper gums when smiling.  Thinning of the upper lip, when smiling, is caused by muscles that elevate, stretch and thin the upper lip.  Neuro-modulators can be used to weaken these muscle in order to decrease their effect.  When smiling, the accepted amount of “gum show” is 2 mm’s above the upper line of the teeth.  Anything greater than that amount is considered to be a Gummy Smile.

Using Botox, Xeomin or Dysport, the upper lip elevator muscles can be weakened by precise application at the convergence point of the three elevator muscles called the “Yonsei point”.

A study in Aesthetic Surgery Journal published in March 2014 found that off-label use of neuro-modulators (Botox, Xeomin, Dysport) was a safe and effective alternative treatment for gummy smile in the right hands.  The downside of Botox, Xeomin for gummy smile is that the effect of these medications lasts about 3-4 months and the treatment has to be repeated to maintain the results.  The cost of Botox for gummy smile is also more appealing than any surgical option.  Treatment costs average $300 around the country and is charged as a professional fee for the treatment instead of per unit due to the expertise required for this type of treatment.  Beauty is more than only skin deep so be sure to find an experienced Botox Injector, like Dr Palmer for correction of your gummy smile.

Chin Implants and non surgical Chin Augmentation by Dr Francis Palmer

I have performed many Chin Augmentations with Chin Implants for over 25 years and when performed properly and the correct aesthetic implant is selected, there are very few things that can go wrong. The main issues to consider when having a Chin Implant surgery are:

  • ​The aesthetic size, shape and thickness (called projection) of the chin implant. There is an aesthetic rule, within my Palmer Code beauty principles that defines the proper aesthetic balance of the chin in a man and a woman’s face that I have used for over 25 years to make this determination.
  • The approach used to place the Chin Implant. There are two ways to place a chin implant but I have always only used the external approach through a small curved incision under the chin. When chin implants are placed through the mouth the attachment of the chin muscle must be cut in order to make the proper space for implant placement. This causes the chin muscle to sag later in life (ptosis) and may also allow the implant to shift into the groove inside of the mouth. I have dealt with both scenarios for chin implants placed elsewhere on numerous occasions, so this can and does happen. When placed through the external approach, the muscle is lifted up but the attachment remains intact.
  • The level of the chin implant placement. It is advised that the implant be placed under the periosteum not within the superficial tissues. This will secure the implant against moving. Screws are not required to secure chin implants.
  • Typically, chin implant surgery can be done in less than 30 minutes. There’s minimal bruising and swelling. There may be some mild restriction of movement/temporary numbness of the lower lip based on the added volume (from the implant) under the chin muscle. This tends to decrease slowly over a few weeks.
  • If an inexperienced surgeon performs the chin implant surgery, there’s an increased risk of damaging the marginal mandibular nerve that can lead to permanent loss of lower lip movement. Like all treatments and procedures the number one goal is to find a plastic and cosmetic surgeon with the proper aesthetics of facial beauty…the second goal is to fine one with a vast experience.

Remember, you can always use a filler (I prefer Radiesse) as a non-surgical method of Chin Augmentation if you’re not ready for a Chin Implant. Over half of our chin and cheek implant consults are not candidates for implants and get fillers instead, so this is a real, viable option.
